Become A Patient at Volunteers in Medicine
Pre-screening begins when the prospective patient reaches out to VIM usually by phone or walking into the clinic. The prospective patient is invited to visit VIM for an appointment with one of the clinic’s trained screeners or the social worker. During this visit, the individual completes paperwork and provides income information to determine eligibility of services.
To be eligible to become a patient at VIM, you must:
- Be employed, actively looking for employment, or receiving unemployment benefits
- A household income at or below 200% of the annually published Federal Poverty Guidelines (see chart below)
- NOT have private health insurance
- NOT have federally funded insurance such as Medical Assistance
- Live or work in Luzerne County
To determine eligibility, prospective patients must provide appropriate financial and other documentation during a scheduled screening appointment:
- Proof of identity
- Proof of work status and household income (recent pay stubs, a recent W-2 form, an employer's verification of wages and a tax return)
- Proof of residence
